Organizational measures are implemented at the Dr. Ernesto Guevara General Teaching Hospital to make services more efficient and provide better care, as part of the transformations in the sector and for the control of outbreaks in this health environment.
Las Tunas, Cuba.- Dr. Marianela Zapata Romero, director of the main care center in the province, asserted that they are engaged in the rescue of hospital discipline, essential for compliance with various provisions.
What is the current protocol?
“After the COVID-19 pandemic, the prohibition of hospital visits was in force in the Health System; for this reason, the monitoring of some patients was designed, which is a medical decision defined according to the disease in question and the service, of which Intensive Care, Perinatology, and Cardiology ward -in the case of serious condition- do not require accompanying persons.
“We have conceived two moments for the change of companion, from 6:00 to 8:00 in the morning and at the same time at night, in pursuit of the organization of the rest of the movement of hospital life; i.e., cleaning, visitor's pass, the teaching of students in training and patients resting, very necessary in our environment.
“These hours allow the flow of the institution through the independent entrance, where the Respiratory Emergencies service was previously located, and make it possible to organize daily access of people, with a circulation from eight to 10 thousand among workers, students, family members, and the different services that are provided.
What has been decided regarding food, and compliance with the diet in correspondence with the clinical picture?
“We have the diet for the various risk groups, there is complete protection in this regard. The decompensated patient and those in a certain state of severity, or disease, must comply with what corresponds.
"To collaborate in the food process that the family reinforces, especially fruits, natural juices, and vegetables, we are opening a strategy that will allow from 12:00 to 1:00 in the afternoon, through the same staircase to change companions, that the population brings a bag well identified with the bed and the room of their relative. There, some authority of the center (managers, deputy directors, heads of service, ward, area...) will make that shipment reach its destination.”
"In this rescue of hospital discipline, the support and understanding of the population is very important, the custodians cannot allow access to all the spaces and they must check the belongings when leaving the hospital since the extraction of institution resources has been detected.
"Regarding clothing, there are regulations as in other places. Entry is not allowed in shorts, T-shirts, or short clothes, you must speak in a low voice, and the companion must not wander or smoke on the sides of the rooms."
“We limit the use of telephones because loud music disturbs the other patients; besides, its use to take photos and videos is prohibited, in the cubicle you must have ethics to respect the sick."
And the discipline regarding the control of foci?
“Currently, we recover all the means for hand washing and we demand the use of the facemask, regulations that have not changed since the start of COVID-19. We have prohibited access with respiratory symptoms since there have been outbreaks in some rooms caused by companions."
“The issue of the rescue of accompaniment in childbirth, an opportunity that the family enjoys and that has a preparation from the Health area, is very important. This requires strict measures so as not to interfere with the dynamics of the place and whoever assists in support cannot present respiratory symptoms."
“Until now, we have implemented prenatal support with a whole strategy from Primary Health Care, so that the family feels better, participates in that significant moment, and transmits peace of mind to the pregnant woman and thus superior results are achieved.
“We are also raising the quality of the information for the population about patients, there is a large location, recently repaired, with many conditions. We need the relatives to have a little more confidence and remain in that place."
“The doctors go there at 1:00 in the afternoon to carry out the interview accurately. We are incorporating other services such as Maternity, specifically Obstetrics, in this detailed report, a very sensitive area that does not have follow-up, and which is a prioritized program at the national level.”
